I created and administered the survey shown below in 2018, shortly after assuming oversight of a library division. The survey results helped me develop my initial priorities in this leadership role. Although I designed the instrument for use in a library, it can easily be adapted for use in another setting. Survey items deal with the following aspects of life in the organization:
Understanding of role
Relationship with management
Workload
Motivation
Future orientation (innovation, learning, and optimism)
Communication and collaboration
Attrition
